Clear, thorough, and compliant reports can make a big difference in supporting NDIS participants to access and maintain the services they need. Our dedicated physiotherapy report-writing service is designed to save time, reduce stress, and provide high-quality documentation for plan reviews.
We aim to make the process straightforward and flexible:
Agreement
We begin with a service agreement outlining the scope of the report, costs, and timelines.
Initial Assessment (Telehealth or In-Person)
An initial assessment ensures we understand the participant’s needs, functional abilities, and goals. Telehealth is often a convenient option, but face-to-face can be arranged if required.
Gathering Information
We collect the key details needed to prepare the report:
Personal details (participant name, NDIS number, date of birth)
NDIS plan information (plan dates, stated goals)
Key contacts (plan manager, support coordinator, allied health providers)
Details of condition and relevant history (diagnosis, onset, current status)
Functional needs and challenges (mobility, self-care, community access)
Report Preparation
We compile a clear, evidence-based report that links assessment findings with the participant’s goals and NDIS criteria.
Each report is tailored to the participant’s needs and the NDIS purpose:
Participant background and medical history
Current function (mobility, strength, endurance, daily activities)
Impact of condition on everyday life
Progress towards current plan goals
Recommendations for supports or equipment (e.g., assistive technology, therapy frequency)
Rationale for funding requests
Alignment with NDIS language and outcome measures
This approach ensures our reports are clear, structured, and ready for NDIS submission.
NDIS reports are often required for access, plan reviews, or ongoing support — but they can be time-consuming to prepare and difficult to get just right.
By working with a dedicated physiotherapy team specialising in NDIS reports:
You save time and reduce stress — no need to chase reports at the last minute.
Reports meet NDIS standards — improving the chance of smooth approvals.
Information is accurate and evidence-based — grounded in professional assessment.
Flexible access — assessments can be done via telehealth or in person.
Whether you’re a participant, support coordinator, or provider, our service helps ensure that high-quality reports are available when they’re needed.
To prepare a high-quality NDIS physiotherapy report, we rely on accurate and detailed information. This includes personal details, medical history, and a clear understanding of current function. Progress reports or assessments from existing physiotherapists or allied health providers are extremely valuable, as they provide a clear picture of progress and ongoing needs.
Typically, we require approximately one hour for assessment (telehealth or in-person) plus three hours for report preparation to ensure the final document is thorough and compliant.
If you would like to arrange a report or discuss whether this service is right for your needs, we encourage you to get in touch. We are happy to answer questions, explain the process in more detail, and provide guidance on the type of report that would best support your NDIS plan or review.
Funding and Fee Schedule
We offer services under NDIS (Plan-Managed) or private funding. Please get in touch to discuss your needs and fees for service. Our fees are inline with the rates set out by NDIS (https://www.ndis.gov.au/providers/pricing-arrangements). If you have any questions, please contact us to discuss you needs.
Service Agreement
We are required to enter into a Service Agreement with every NDIS Participant who accesses our services. This agreement provides an overview of the estimated services to be delivered for the duration of the participants plan. We will require some details of your NDIS plan on your first visit. This information can be emailed or provided in the initial consultation. Payment for services will be required at the conclusion of each treatment session.
