EPC for physiotherapy is a Medicare-supported program in Australia, now officially called the Chronic Disease Management (CDM) plan, that allows eligible individuals to receive up to five subsidised physiotherapy sessions per calendar year.
To access it, have your GP assess your eligibility, and receive a referral for a CDM plan from your GP.
This plan covers up to five sessions for allied health services, and these sessions are shared across all services.
How to access the program
Consult your GP: Discuss your condition with your GP and ask if you might be eligible for an EPC/CDM plan.
Meet eligibility criteria: Your GP will assess if you have a chronic condition (one that has lasted or is expected to last for six months or more) and need a care plan.
Receive a referral: If you are eligible, your GP will create the CDM plan and provide a referral to a physiotherapist.
Book your appointment: Book an appointment with a physiotherapist who accepts EPC/CDM referrals.
Process the plan: Bring your referral and any relevant documents to your appointment so the clinic can process the plan with Medicare.
Key details about the program
Session limit: You can claim up to five physiotherapy sessions per calendar year.
Shared sessions: The five sessions are shared across all allied health services on your plan (e.g., podiatry, dietetics).
Chronic conditions: Examples of eligible chronic conditions include arthritis, diabetes, back pain, heart disease, and respiratory illnesses.
Costs: All services we provide for an EPC are Bulk-Billed. You have nothing to pay.
